PART III - Release

The Release and Waiver is found on the hard copy of the Registration form. Signature of applicant or signature of applicant's parent/guardian (if under 18 years of age) is required on the hard copy. Therefore, this portion must be completed upon your first attendance to class.

Tuition Fees

Fees will be collected on the first class of Term I. Subsequent fees are due at the end of the preceding term.

HULA – LEVEL I	$55.00 per Term (7 classes)
	$220.00 for the school year (4 Terms)
	Discounted to $200.00, if full payment is received by September 30th, 2008.

HULA – LEVEL II & III	$65.00 per Term (7 classes)
	$260.00 for the school year (4 Terms)
	Discounted to $238.00, if full payment is received by September 30th, 2008.

PERFORMING GROUP	$280.00 for the school year
	Discounted to $255.00, if full payment is received by September 30th, 2008.
